This Service Agreement ("Agreement") is entered into between [Client's Full Name], hereinafter referred to as the "Client," and Polar Cold Caps, hereinafter referred to as the "Company." This Agreement pertains to the use of Polar Cold Caps' products and services for hair preservation in conjunction with the support of a medical team.

1. Purpose of Agreement:

The Client acknowledges that Polar Cold Caps' products are intended to be used as an adjunctive method in reducing hair loss during certain medical treatments, such as chemotherapy. The Company makes no guarantees about the success or effectiveness of the cold cap treatment in preserving hair.  The Client agrees to consult with their medical team before using Polar Cold Caps' products and to follow their recommendations.  The Client understands that using Polar Cold Caps' products involves potential risks and discomfort, including but not limited to cold-related discomfort, headaches, skin reactions, and potential ineffectiveness in preserving hair. The Client assumes all risks associated with using the products.

In consideration of the use of Polar Cold Caps' products, the Client agrees to release, discharge, and hold harmless the Company, its officers, employees, agents, and affiliates from any and all claims, damages, liabilities, expenses, or causes of action arising out of or relating to the use of the products, including any adverse effects or hair loss experienced by the Client.

 Canada Polar Cold Caps Canada will share your information with Polar Cold Caps in order to fulfil your order.
